When to use Modifier 59

When billing Modifier 59 to Medicare please take this into consideration:

WPS Medicare’s system only accepts Modifier 59 on procedure code pair combinations found in the National Correct Coding Initiative (NCCI) edits. You should only use modifier 59 as a last resort.
The Centers for Medicare & Medicaid Services (CMS) published an article on the appropriate use of Modifier 59. To read the article, visit
http://www.cms.hhs.gov/NationalCorrectCodInitEd/Downloads/modifier59.pdf on the CMS Website.
